Why Yamamoto didn't heal his arm in Bleach, explored
The death of Genryusai Shigekuni Yamamoto might be one of the biggest losses for the Soul Society in Bleach, given that he was the face of this organization. Yamamoto's downfall began when he sacrificed his left arm in hopes of capturing Sousuke Aizen during the Fake Karakura Town arc. Moreover, the Captain-Commender never opted to get his hand healed after this incident.
High-level healers like Orihime and Unohana were more than capable of healing his hand, but Yamamoto refused to get his hand regenerated. The reason behind this might be simple. Being the leader of the Soul Society, Yamamoto considered himself responsible for Aizen's betrayal. Unable to confront the antagonist himself, he considered the loss of his left arm as a form of punishment.

Bleach: Exploring why Yamamoto never got his left arm healed
Genryusai Shigekuni Yamamoto was the former captain of the Gotei 13 and the Captain-Commander of the Soul Society. As the oldest captain of this organization, the old man was respected by everyone due to his experience as a Soul Reaper. While he tried his best to maintain peace in the Soul Society, some people opted to disrupt it.
A prime example of this was Aizen Sousuke, the former captain of the Fifth Division. After faking his death, Aizen expressed his desire to replace the Soul King and take over the entire Soul Society. In response, the Soul Society created a Fake Karakura Town (during the Fake Karakura Town arc of Bleach) to lure Aizen in hopes of catching him for good.
However, Aizen came prepared as he brought the Wonderweiss. These artificially created Quincies were specialized to suppress the Zanpakuto of Yamamoto, Ryūjin Jakka. So, the Captain-Commander had to go beyond his powers to subdue the calamity.
As expected, the majority of the battle was fought between Yamamoto and Aizen's allies, as other members of the Soul Society couldn't do much with their powers. Unfortunately, Yamamoto's body was severely injured during his battle against the Wonderweiss, especially his left arm (which was burned to a crisp).
During a final showdown with Aizen, Yamamoto used his burned left arm as a catalyst to activate Hadō #96, Ittō Kasō (a Kidou spell). As a result of this attack, Yamamoto lost his left arm, but unfortunately, Aizen didn't get any noticeable injuries due to this. Later on, Aizen was defeated by Ichigo and imprisoned by Muken for his betrayal.
After this fight in Bleach, Yamamoto chose not to have his lost hand regenerated by Orihime's healing powers. If he had done this, he might have regained his full potential as the strongest. However, he refused, which became one of the key reasons for his defeat against Yhwach in the Thousand Year Blood War arc.
So why did he do such a thing? As the leader of the Soul Society in Bleach, Yamamoto felt responsible for protecting the Soul Reapers and everything related to this organization. Since he couldn't protect everyone, he might have considered the loss of his left hand as a punishment for his negligence.
While this might be overkill, because Aizen's betrayal was planned a long time ago and Yamamoto had no direct connection to it, the Captain-Commander considered it his duty to protect the Soul Society. Thus, he opted not to get his left arm regenerated as a punishment for not bringing justice to the rebel Aizen Sousuke.
Final thoughts
Symbolism is a common theme in anime, and before the tale of Yamamoto, another character didn't get healed for a similar reason. After the Fourth Great Ninja War, Sasuke Uchiha chose not to get a prosthetic hand because he viewed it as a punishment for all the trouble he brought to the Hidden Leaf Village. Both he and Yamamoto shared the same thinking.
