There are a few ways through which you can get the helmet, but each one is very grindy and somewhat tedious. So, get ready to complete these tasks multiple times if you want the gear:

Farming Ladder and Uber Bosses

Farming Ladder Bosses and Uber Bosses offer a chance to farm Mythic Uniques like the Heir of Perdition. The specific bosses you should focus on are:

  • Lord Zir
  • Grigoire
  • Echo of Duriel
  • The Beast in the Ice
  • Echo of Andariel
  • Varshan

Grind these bosses repeatedly until you get the drop you desire. The chances of you getting the item on the first try are extremely low, but you will get some other good loot each time you defeat them.

Transmuting with Resplendent Sparks at the Alchemist

If you're struggling to get the Heir of Perdition as a drop, you can craft it through Transmutation:

Ingredients Needed:

  • Gar x6
  • Que x6
  • Jah x6

or

  • Resplendent Sparks x2

Visit the Alchemist in Diablo 4 and use the above materials to craft the helm. This method is especially useful if you have the necessary runes and sparks on hand because, let’s be honest, finding runes is just as tedious as farming bosses.

Gambling at the Purveyor of Curiosities

Lastly, you can try your luck by gambling for Mythic Uniques at the Purveyor of Curiosities using Obols:

  • The odds of getting a Mythic Unique are extremely low, but it's a viable option if you have an abundance of Obols to spend.
  • Keep in mind that this is not a guaranteed method and can easily eat up all your resources.

  • Succumb to hatred and earn Mother's Favor (Lilith’s Favor), increasing your damage dealt by 60%. Slaughter enemies to briefly steal Mother's Favor from nearby allies.

This effect simply means that if you wear the helmet, you will get a 60% boost to your damage. If there are other players around who are also wearing this helmet, you will have a chance to steal their 60% damage boost and add it to yours if you kill them. Essentially, this means your damage boost will increase to 120%.
